The strongest correlation to cognitive and physical disabilities and brain atrophy in MS is the amount of iron deposition in the MS brain. Not white matter lesions, but T2 hypo-intensities showing iron in the MS brain. Even in Clinically isolated syndrome (CIS), T2 hypo-intensities show up before lesions.
