Summary
CCSVI (Chronic Cerebrovascular Venous Insufficiency) is still an open issue for discussion and is one topic that is still object of controversy. Easy to say, the discussion is open principally to have this pathology recognised as a reality, to give it a scientific identity. Quite obviously it is easier to demonstrate what is universally accepted rather than what is subject to demonstration. The latter is to accept, or not, anatomical and/or physical adverse conditions for venous outflow from the cephalic and spinal chord area and therefore approve eventual operations to correct the anomalies, according to their number and distribution. Still, things are moving and this because of results, which mean numbers.
Numbers are made of variables, the less the better and this is perhaps the weak side for CCSVI diagnosis and consequent results.
The diagnosis, as most experts know, is principally based on Ultrasound Duplex exams, where the dedicated machine is prepared on the purpose. The Physician diagnostician as well, is prepared for the specific exam that is carried out following Zamboni’s parameters, both in lying and sitting positions.
The weak ring of this imaginary chain are the patients; they are not all the same, like their disease - Multiple Sclerosis (MS)- that is under the highlights of this specific type of diagnostic exam. As a consequence also the results quite frequently are different.
This is a sort of cascade that necessarily cannot easily transform both the diagnostic exam, and the results into certainty.
These affirmations are possible only because there is a personal heavy background made of numbers and a sufficient amount of time to grow these numbers. Moreover, a certain number of papers oppose CCSVI even if, and this too should be highlighted, most of these works have an elevated interest conflict rate. In time and with the conclusion of Zamboni’s “Brave Dreams” study, more exact information will be given on this pathology. — met Jorge Mashini en 24 anderen.
