"Optimism, Enthusiasm, Responsibilty" by Gregory L. Moneta, MD, Portland, Ore

"However, clearly, enthusiasm remains at one Italian
center. Whereas the authors can be criticized for treating
hundreds of patients without rigorous documentation of
clinical or sustained anatomic benefit, they have done a
service by pointing out some anatomic features of cerebral
venous drainage that suggest a better initial
response to venous angioplasty. Because there can be
no long-term patency without short-term patency,
perhaps they are helping the CCSVI enthusiasts by identifying
patients most likely to have a good technical
result with venous angioplasty and thereby identifying
who should be included in clinical trials of venous angioplasty
for MS to maximize the possibility of showing
benefit. With this, perhaps the CCSVI enthusiasts have
one more bit of information to help them do the responsible
thing and perform and report proper trials. So far,
this also has not happened"
